Enrolment Information
Birth Certificate and Immunisation Documents
As part of the normal enrolment process the NSW Department of Education requires the parent/carer to provide the following original documents when a student is enrolled in a NSW Government school.
These original documents will be copied and returned to you.
These documents include:
· Birth Certificate
· Immunisation history statement
· Proof of residential address
· Copies of any family law or other relevant court documents (if applicable)
